2. Using your invoice, indicate the TOTAL AMOUNT DUE at the top, from your invoice.
3. Under PATIENT NAME OR ACCT NO, indicate the PATIENT’S FULL NAME or PATIENT ACCOUNT NUMBER.
4. Fill out your payment information to complete the transaction.
